You can print this page on your printer, fill in the nesessary shipping information and send it to the Video Addicts address on the form. Be certain, to accurately calculate your shipping costs. Enclose a check, money order or concealed cash (i.e., wrapped in carbon paper), make all funds payable in US$ Dollars, to Dave Masters. Put a stamp on it, mail it, you're done.
- Word Processor Method
Okay, so you don't have access the Internet 24/7. In this case, take down all pertinent information about the video(s) or DVD(s) that you want, being certain to note the Order Number, the format, the full title and shipping charges, before you begin.
